This package is a structured two-visit process designed to identify the factors affecting your fertility and create a clear plan for next steps.
During your first visit, we’ll review your full history and determine which testing is needed.
At your second visit, we’ll go over your results and outline a personalized plan moving forward.

Your first visit is 75 minutes and completed via telehealth.
A credit card is required to reserve your appointment.
A $200 non-refundable booking fee is charged at the time of scheduling and is applied toward the total cost of the Root Cause Fertility Diagnosis Package ($1,600).
The remaining balance will be charged two business days before your visit.
Lab testing is billed separately.
After booking, you’ll receive an email from Charm with instructions to create your patient portal and complete your intake paperwork.
