We’ll start with the Fertility Diagnosis Package – please go ahead and use the scheduler below to book your first 75-minute appointment. The package includes two (2) visits (see the FAQ section for more information) – we will book that second, follow-up visit together once we have your lab results.
You will be required to put a credit card on file, and we will charge a NON-REFUNDABLE booking fee of $200. That cost ($200) will be credited towards the total cost of the Fertility Diagnosis Package ($1600), the remainder of which will be charged two business days before your scheduled visit.
You’ll receive a confirmation email (check your spam) from Charm PHR (patient health records), and you’ll be prompted to create a Charm portal account and fill out paperwork in preparation for the visit. You’ll also receive a text from our office welcoming you to the practice and giving you any additional information you need.
